 Contact the nearest VA Regional Office, national cemetery,
local veterans' organization or library for forms.
 

VA Form 40-1330 Eligible: Any deceased veteran discharged under conditions other than dishonorable. Must provide an official document pertaining to military service, ie. muster rolls, extracts from State files, pension or land warrant.
